Charity Wish List Submission Form

One of the primary objectives of the Community Foundation of West Georgia is to increase philanthropy by making it easy for people who want to make a difference, connect to various community needs. We are happy to help promote charitable giving, whether it is through the Community Foundation or directly to a charity. Needs submitted for listing on our website should address a significant need affecting a broad segment of the region's population or special populations considered at risk. This opportunity is open to any non-profit agency which serves the Carroll, Haralson or Heard county areas.

Instructions
1. Complete the information below. Be concise and specific.
2. Consider submitting needs with a range of "price tags"; a donor who makes a $100 gift now and has a goodexperience may eventually make a larger gift.
3. Submit your completed form in one of the following ways:

Or by completeing the online form below.

4. Keep the Foundation informed as needs are met, particularly if they are met as a result of being listed on ourwebsite.
5. Understand that donations requested must be tied to a specific program or project (i.e. no items are to berequested for use at fundraisers or auctions; no donations should be requested for general operating purposes).
6. Update your listing as needs change by one of the choices listed in #3.
